NY prisons struggle: Accountability, violence, understaffing

The New York State Department of Corrections and Community Supervision has released two reports detailing failures in state prisons, including violence, understaffing, and lack of accountability, as well as a "us versus them" mentality between staff and incarcerated individuals.
